GMST requires medical information for participation. Since most of our athletes wish to participate in the Special Olympics GA (SOGA) state games, we use the SOGA medical form.  Please use the new 2023 multi-page form which you can find here:

Please be sure to send all forms to GMST, not directly to SOGA.  We will make sure it gets to SOGA and retain a copy.
